Key Responsibilities
- Familiarity with CMS Regulations and NCQA HEDIS Guidelines;
- Understanding of Evidence-based Clinical Practice Standards, including those from the American Diabetes Association (ADA), American College of Cardiology (ACC), and American Heart Association (AHA);
- Conduct comprehensive and accurate evaluations of the assigned member population, ensuring timely documentation submission;
- Document all relevant medical conditions in compliance with CMS/DHHS and client requirements;
- Facilitate effective communication between physicians and patients, ensuring all assessment findings and recommendations are documented appropriately;
- Provide general advice to members to enhance their understanding of chronic conditions and necessary testing;
- Identify and address gaps in care, whether disease-specific or preventive;
- Recognize urgent situations that require escalation and respond appropriately;
- Perform and document results of Point of Care (POC) Testing;
- Adhere to all Inovalon policies and procedures, ensuring compliance with confidentiality and HIPAA regulations;
- Fulfill additional responsibilities as assigned to support the operational success of the organization;
- Maintain the separation of duties as applicable within your role;
- Other duties as required.
- Current, unencumbered nursing license;
- Nurse Practitioner Certification from ANCC or AANP;
- Current CPR certification (ACLS, BLS - Provider, CPR - Provider or ATLS);
- Compliance with recommended vaccinations and preventive testing prior to hire;
- Specialization in geriatric or adult medicine is advantageous;
- Ability to communicate effectively with elderly and chronically ill patients and their families;
- Understanding of Medicare, Medicaid, and Health Plan benefit structures is beneficial;
- Strong multitasking abilities;
- Exceptional customer service skills;
- Fluency in multiple languages is a plus.
