Key Responsibilities- Provide physical therapy services as prescribed by physicians to patients in their homes
- Conduct initial evaluations and develop personalized treatment plans to address patients' specific needs
- Collaborate with a multidisciplinary team to coordinate patient care and optimize treatment outcomes
- Supervise physical therapy assistants and educate patients and caregivers on proper use of equipment and devices
- Maintain accurate and thorough documentation of patient progress in compliance with regulatory requirements
- Current licensure to practice Physical Therapy in the state of employment
- Possess and maintain a valid Driver's License
- Ability to work independently while adhering to established clinical protocols and guidelines
- Excellent oral and written communication skills
- Excellent organizational and time-management abilities to effectively manage caseloads and meet deadlines
