Job Summary:

Community Care Home Health is seeking a skilled and compassionate Home Health Aide to provide personal care and support to patients in their homes. As a Home Health Aide, you will work under the direct supervision of a licensed RN to assist patients with daily living activities, such as bathing, grooming, and toileting. You will also promote a safe environment, maintain unobstructed pathways, and assist with ambulation, transfers, and range of motion exercises.

Responsibilities:
  • Assist patients with activities of daily living, such as bathing, grooming, and toileting
  • Promote a safe environment and maintain unobstructed pathways
  • Assist with ambulation, transfers, and range of motion exercises
  • Document changes in patient condition and report to supervisor
  • Attend mandatory company in-services and maintain current employee files

Requirements:
  • Home Health Aide certification obtained through successful completion of a state-approved training program
  • Ability to effectively communicate with patients and staff
  • Ability to prioritize multiple tasks and deal with changing patient needs
  • Ability to work in a fast-paced environment and adapt to changing situations

Working Conditions:
  • Work is normally performed in a residence or senior living facility
  • May involve frequent exposure to communicable diseases and other potentially infectious materials
  • Ability to travel from assignment to assignment within the geographic area served

Physical Effort:
  • Medium physical activity that may require constant lifting, carrying, pushing, or pulling over 50lbs
  • Ability to stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs, and reach freely
