Nurse Practitioner (Skilled Nursing)

As a key member of our interdisciplinary team, you will provide high-quality internal medicine services on Saturday and Sunday at our Skilled Nursing Facility. Your expertise will be instrumental in reducing hospitalizations on weekends and ensuring the best possible outcomes for our patients.

Key Responsibilities:
  • Evaluate new-admission visits and coordinate discharge visits
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with attending physicians and/or specialists regarding patient care
