Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at HealthONE Hospice & Family Care. As a PRN LPN, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify the physical, psychosocial, and environmental needs of patients, documenting data in patient medical records.
  • Communicate significant findings, problems, or changes in patient condition to the supervising RN and/or Director of Professional Services and the physician, documenting all findings and interventions.
  • Provide basic and specialized teaching as ordered in the Plan of Care and based on patient needs, identifying and recommending other disciplines as necessary for quality care.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Licensed Practical/Vocational Nurse (LPN/LVN) licensure.
  • Valid driver's license.
  • Vocational school graduate.
