Job Description:

  • Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits and participate in periodic office meetings.
  • Manage a caseload of 58 patients per day.
  • Complete documentation within 24 hours of patient appointments.
  • Achieve weekly productivity expectations of 30 points.
  • Be open to floating to nearby clinics if patient census is low.
  • Participate in weekly clinic meetings.
  • Maintain complete and timely medical records for each patient.
  • Efficiently plan travel routes to optimize time and minimize travel between patient homes.
