Responsibilities:- Provide nursing care to residents in a variety of settings, including long-term care and rehabilitation.
- Assess residents' physical and psychosocial needs and develop individualized care plans.
- Administer medications and treatments as ordered, and document resident interactions and outcomes.
- Collaborate with healthcare professionals to provide high-quality care and support.
- Communicate effectively with residents, families, and healthcare teams to ensure seamless care transitions.
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
- Obtains and subsequently maintains required department specific competencies and certifications.
