We are seeking a skilled LVN/LPN nurse to join our team in a rehabilitation and long-term care setting. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Responsibilities:
  • Provide nursing care to patients in a rehabilitation and long-term care setting, as directed by the physician.
  • Administer medications and document therapeutic effects.
  • Conduct and document assessments of patient medical status during the course of care and/or treatment.
  • Perform all duties in accordance with practice policies and procedures.
  • May be responsible for performing administrative duties and rotating as a triage nurse.
Qualifications and Job Info:
  • 1-2 years of recent experience in a rehabilitation and long-term care setting.
  • BLS certification.
  • ACLS certification preferred.
  • Active state license.
