Nurse Practitioner

Traditions Health is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

• Conduct comprehensive assessments of patients' physical, emotional, and social needs
• Develop and implement individualized care plans to meet patients' unique needs
• Provide skilled nursing care, including wound care, medication management, and education
• Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
• Monitor patients' progress, adjust care plans as needed, and document outcomes

• Current State license as a Registered Nurse
• One year of experience as a Registered Nurse in a clinical care setting
• Home health experience preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
