Case Management RN Opportunity

We are seeking a skilled Case Management RN to join our team at Providence St. Mary Medical Center in Apple Valley, California. As a Case Management RN, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' medical and psychosocial needs
  • Develop and implement individualized care plans to achieve optimal patient outcomes
  • Collaborate with healthcare teams to ensure continuity of care and effective communication
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
Requirements:
  • Active and unencumbered California RN license
  • At least 2 years of current experience in case management or a related field
