Patient Care Specialist Opportunity

Penn Medicine is dedicated to delivering exceptional patient care and innovative research. As an Occupational Therapist - Patient Care Specialist, you will collaborate with our interdisciplinary team to provide optimal patient experiences and meet patient needs. Your expertise in occupational therapy will help patients maintain, improve, or restore function to daily living activities and instrumental activities of daily living.

Key Responsibilities:

  • Collaborate with the interdisciplinary team to develop and implement individually designed programs of occupational therapy treatment.
  • Evaluate patient data to identify functional impairments and manage caseloads to ensure timely therapy revisits and reassessments.
  • Confer with patients, family members, caregivers, physicians, and multi-disciplinary team members to plan, implement, and assess intervention programs.
  • Administer ADL/IADL retraining, therapeutic exercises, and home safety training to improve patient function and quality of life.
  • Instruct patients, family members, and caregivers in treatment procedures to increase carry-through with the plan of care and improve patient outcomes.
  • Perform medicine reconciliation and monitor patient vital signs to communicate findings to team members and physicians.
  • Discharge patients from occupational therapy when goals or projected outcomes have been attained or patient is no longer homebound.
  • Assign patients to occupational therapy assistants as appropriate and provide supervision according to the OT practice Act and PCAH policies.

Requirements:

  • Bachelor's Degree in Occupational Therapy from an ACOTE-accredited school of occupational therapy - Master's or Doctorate preferred.
  • Pennsylvania OTR/L license.
  • BLS/CPR through AHA.
  • Valid Driver's License.
  • Car Insurance.
