Licensed Practical Nurse

Responsibilities:
* Perform assessments and simple nursing procedures in client's homes, under the guidance of the RN Supervisor
* Implement nursing care based upon the plan developed by a Registered Nurse (RN) in accordance with physician's orders
* Provide patient and family teaching according to plan outlined by the RN
* Monitor patient's vital signs and make specific measurements and observations
* Monitor and record intake and output, reporting significant changes immediately
* Administer medications as ordered by the physician and instructed by the RN
* Document the patient's file accordingly
Requirements:
* Must be a graduate of an accredited school of practical nursing
* Must be licensed as a vocational/practical nurse in the state of practice
* Must have at least one year clinical practical nursing experience, preferably in intensive care, rehabilitation, or medical surgical nursing
* Must be able to make home visits and provide nursing care in the home environment
* Must be able to complete necessary documentation appropriately, legibly, and in a timely manner
* Must have interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Working Conditions:
* The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job
* While performing the duties of this job, the employee is regularly required to sit, talk, hear, and to use hands to finger and handle controls
* The employee frequently is required to reach with hands and arms
* The employee is occasionally required to stand, walk, bend, twist, reach, and stoop
* Specific vision abilities required by this job include close vision and the ability to adjust focus
