Charleston, West Virginia, United States Signify Health Full time

Unlock a rewarding career with Signify Health, part of CVS Health, where we strive to build trusted relationships and make people healthier.

Salary and Benefits

As a board-certified nurse practitioner contractor, you can earn up to $2,500 to $4,000 each week by conducting in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

  • Average 6-9 visits daily, base rate of $80 per completed assessment
  • Increase your per-visit rate up to 135% or 160% (depending on working counties) with our Market Multiplier*
  • Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)

We offer flexibility and independence as an independent contractor, 1099 status. You'll have short-term or long-term basis options and pre-scheduled visits in-home with health plan members. We provide malpractice coverage, availability incentives, and flexible scheduling.

Job Description

Our nurse practitioner contractors conduct in-home visits that focus on a person's overall health and well-being, identifying potential barriers to care. These visits include medication and medical history reviews, physical evaluations performed in a health plan member's home, and providing pre-populated patient-specific forms for each evaluation.

  • No prescribing medicine, ordering lab tests, or altering people's current treatment regimen
  • Flexibility to manage other personal and clinical commitments

You will be empowered to form connections with people, educate them about their treatment plans, and assess their understanding without time restraints found in traditional office settings. Our goal is to empower people to become better advocates of their health.

*Incentive rates are subject to change. Some exclusions apply.

Why NPs Love Working with Signify Health

Our nurse practitioners appreciate the benefits of in-home visits, including more time to ask questions, educate patients, and assess their understanding of their treatment plans. This setting allows us to connect with people on a deeper level and make a meaningful difference in their lives.

Signify Health offers a unique opportunity for nurse practitioners to grow professionally and personally while making a positive impact on the lives of our health plan members.
