  • Link, monitor, and coordinate services to meet the unique needs of each child and family.
  • Develop and implement Person-Centered Plans for assigned individuals, taking into account their strengths, needs, and preferences.
  • Advocate effectively for individuals served, ensuring their voices are heard and their rights are respected.
  • Monitor and document outcomes, revising plans as needed to ensure the best possible results.
  • Support culturally competent, recovery-based practices that prioritize the well-being and empowerment of individuals and families.
Requirements:
  • Bachelor's Degree in a Human Service-related field.
  • Licensure as a Social Worker in Michigan or qualifications to obtain (preferred).
  • Valid Driver's License.
  • Minimum of one-year clinical experience with the population served (preferred).
  • Meets QIDP (Qualified Intellectual Disability Professional) qualifications (preferred).
Preferred Skills and Abilities:
  • Positive communication and effective teamwork.
  • Ability to manage multiple tasks and priorities.
  • Knowledge of appropriate resources for eligible consumers.
  • Bilingual (Spanish) skills preferred but not required.
