We are headquartered in Nashville, TN, and currently operate in Massachusetts, Delaware, Vermont, New Hampshire, Tennessee, Maine, and Virginia.

Primary Responsibility:
A qualified Registered Nurse will offer comprehensive care management, care coordination, health promotion, transitional care, individual and family support, and referrals to community services for patients with Substance Use Disorder. Medicines prescribed are primarily Buprenorphine and Vivitrol.

Job Description:
  1. Care Management:
  • Activities undertaken to identify patients for Medication Assisted Therapy.
  • Conduct initial assessments working with the site team.
  • Help formulate individual plans of care.
Care Coordination:
  • Implementation of individual plans of care through linkages, referrals, coordination, and follow-up.
  • Coordinate services across treatment and human services settings and providers.
Health Promotion:
  • Promote patient activation and empowerment.
  • Support healthy behaviors and self-management of health, mental health, and substance abuse conditions.
Transitional Care:
  • Streamline movement of patients between treatment settings.
  • Reduce hospital readmissions and facilitate timely development of community placements.
Individual and Family Support:
  • Assist patients in accessing care.
  • Support age and gender appropriate adult role functioning and promote recovery.
Referrals and Community Services:
  • Facilitate connection of patients to informal community resources.
  • Ensure all policies and procedures related to patient engagement in treatment are upheld.
