Job Summary: The Patient Care Coordinator - Case Manager will work closely with our interdisciplinary team to provide high-quality patient care. This role requires strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Deliver skilled nursing care to patients in their homes
  • Educate patients and their families on disease management and treatment options
  • Create and implement personalized care plans
  • Collaborate with other healthcare professionals to ensure comprehensive care

Salary: $83,000 - $120,000 per year, depending on experience.

Benefits: We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.

Requirements:

  • Bachelor's degree in Nursing
  • Current RN licensure in Vermont
  • Minimum 2 years of experience in hospice or home healthcare

Incentives: $10,000 Sign On Bonus for full-time external applicants, prorated for part-time roles. Additional terms and conditions apply.
