The nurse will assess physical, psychosocial, and learning needs of patients and families, and serve as the professional nurse by assessing, planning, directing, practicing, and evaluating patient care in a designated group practice.

Responsibilities:
  • Provide direct patient care and administer medications as prescribed by physicians.
  • Assess physical, psychosocial, and learning needs of patients and families.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare teams to ensure seamless care coordination.
  • Counsel patients and families on health education and disease management.
Requirements:
  • Associate's degree in Nursing.
  • Current licensure to practice as a Registered Nurse in the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
  • Basic Life Support (BLS) certification.
