As a Nurse Care Specialist with IntelyCare, you will have the opportunity to work in a variety of post-acute care settings, including Assisted Living Facilities (ALFs) and Skilled Nursing Facilities (SNFs). Your primary responsibility will be to provide high-quality nursing care to residents, working closely with medical staff to develop and implement individualized care plans.

Key Responsibilities:

  • Administering nursing care to residents in post-acute care settings
  • Developing and implementing care plans in collaboration with medical staff
  • Ensuring residents receive the right personalized treatments and medications
  • Explaining treatment options to residents and their families
  • Consulting with other healthcare professionals, including doctors
  • Setting up and monitoring medical equipment
  • Overseeing compliance with facility policies and nursing standards
  • Documenting interventions and medical histories
  • Performing fluid sample draws for lab work
  • Leading training and supervision of nursing staff
  • Assisting with incident investigations and accuracy of reporting

Requirements:

  • Current Ohio state license to practice as a Registered Nurse (RN)
  • Graduation from an approved nursing program
  • Post-acute care or in-patient hospital experience (at least 4 months)
  • Strong organizational skills
