Our Job OpportunityThis is an exciting opportunity for a skilled Nurse Practitioner to join our team in Carlsbad, NM. As a Home Risk Assessment Nurse Practitioner, you will have the chance to leverage your expertise and compassion to make a direct impact on the health and well-being of others.
You will be responsible for conducting comprehensive health assessments in the comfort of patients' homes, using your skills and experience to identify potential health risks and develop personalized care plans.

Responsibilities- Conduct thorough adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up as necessary.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
- Masters degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in state of practice, including prescription authority or ability to obtain prescriptive authority.
- Board certification by AANP, ANCC, or AACN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
