Job Overview:
Aveanna Healthcare is seeking a skilled and compassionate Home Health LPN/LVN to join our team in Camden and Pennsauken. As a key member of our healthcare staff, you will be responsible for providing high-quality patient care, working closely with physicians and other healthcare professionals to ensure the best possible outcomes.

About the Job:
This is a full-time opportunity that requires days and overnights during weekends. You will work directly with patients in their homes, providing hands-on care and support as needed. Your primary goal will be to help patients achieve optimal health and independence, while also ensuring their safety and well-being.

Key Responsibilities:
- Provide and document skilled nursing care, under the supervision of a Registered Nurse, in accordance with developed care plans and physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered.
- Participate, implement, and update the nursing care plan as necessary.

Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
- Continuing Education as required by state

Preferred Qualifications:
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience

Salary Information:
The estimated annual salary for this position is $70,000 - $90,000, depending on qualifications and experience. Aveanna Healthcare offers a competitive compensation package, including benefits and opportunities for professional growth and development.
