About the Role

We are seeking a compassionate and detail-oriented Medication Care Partner to join our team at Cadence Millbrae. As a Medication Care Partner, you will play a vital role in ensuring the health and well-being of our senior residents by providing direct care and support with medication administration.

Key Responsibilities
  • Assist residents with daily medication administration, including self-administration
  • Keep accurate and up-to-date charting documentation and report resident status changes
  • Coordinate medication orders and deliveries with pharmacies
  • Provide compassionate care and support with activities of daily living to residents in a team setting as needed
  • Prioritize independence and provide emotional support to residents by offering continuous engagement and companionship
  • Encourage and assist with participation in life enrichment activity programs
  • Promote open communication between healthcare professionals, families, residents, and staff
Requirements
  • Positive team player attitude and love working with people
  • Excellent verbal and written communication skills
  • A minimum of 1 year of caregiving experience, including giving medication in a community setting
  • Experience with Memory Care is strongly preferred
  • Current caregiver training/certification as specified by the state, or the willingness/ability to obtain
  • Current First Aid and BLS certificate or ability to obtain
  • Previous experience working in Independent Living (IL), Assisted Living (AL), Memory Care (MC), or Senior Living experience is a plus
  • High School Diploma or equivalent
