RN Case Manager Supervisor

About the Role:
We are seeking a highly skilled and experienced RN Case Manager Supervisor to join our team at the University of New Mexico - Hospitals. As a key member of our Outpatient Care Management Services department, you will be responsible for overseeing the initial review, triage, and assignment of patients to case managers, as well as coordinating all systems and services required for a patient-centered care team approach.
Key Responsibilities:
- Develop and implement efficient organizational structures to ensure effective patient care.
- Supervise and mentor staff to ensure high-quality patient care and adherence to hospital policies and procedures.
- Collaborate with interdisciplinary teams to ensure continuity of patient care and promote effective communication among healthcare team members.
- Assess patient needs and develop comprehensive plans of care to ensure quality, cost-effective care.
- Monitor and evaluate patient responses to interventions and implement resolution strategies as needed.
Requirements:
- 3 years of direct experience in case management or a related field.
- Current RN licensure in New Mexico or eligibility for reciprocal agreement.
- CPR for Healthcare/BLS certification.
