About the Role
  • Specialize in providing personalized physical therapy care to adult and geriatric clients in their homes.
  • Work independently, assessing client needs, developing individualized care plans, and implementing interventions to promote optimal health and functioning.
  • Collaborate with multidisciplinary teams, including nurses and other healthcare professionals, to ensure comprehensive care and support.

The ideal candidate will possess strong clinical skills, excellent communication and interpersonal abilities, and a commitment to delivering exceptional patient care.

Key Responsibilities
  1. Conduct thorough assessments of clients' physical function and mobility, identifying areas for improvement and creating targeted treatment plans.
  2. Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and quality of life.
  3. Educate and instruct clients and their families on safe transfer techniques, proper use of assistive devices, and other essential self-care strategies.
  4. Accurately document observations, interventions, and outcomes, utilizing state-of-the-art electronic medical records systems.
Requirements and Qualifications
  • A current Ohio Physical Therapist license.
  • Graduation from an accredited physical therapy program.
  • Strong analytical and problem-solving skills, with ability to work effectively in fast-paced environments.
  • Excellent communication and interpersonal skills, with ability to build rapport with diverse client populations.
