Nurse Care Specialist

We are seeking a dedicated Nurse Care Specialist to join our team of healthcare professionals at Advocate Health. As a key member of our patient care team, you will play a crucial role in providing high-quality, patient-centered care to our patients and their families.
Key Responsibilities
- Assess patient needs and develop individualized plans of care to meet those needs.
- Provide ongoing evaluation of patient care and make adjustments as needed to ensure optimal outcomes.
- Collaborate with other healthcare team members to coordinate patient care and achieve desired outcomes.
- Implement strategies to reduce patient risk and increase patient safety.
- Actively coordinate patient discharge plans and apply evidence-based practice to deliver patient care.
- Provide ongoing evaluation of patient, team, and unit outcomes and take action to improve processes and patient outcomes.
- Demonstrate effective communication, feedback, and conflict resolution skills.
- Promote a culture of safety and identify potential safety issues to prevent patient harm.
- Participate in quality and process improvement initiatives to improve patient care and departmental functions.
Requirements
- Registered Nurse license issued by the state in which you practice.
- Basic Life Support (BLS) certification issued by the American Heart Association.
- Completion of an accredited or approved program in Nursing.
- Typically requires 1 year of experience in clinical nursing.
- Proficient computer skills and ability to use electronic medical records systems.
- Excellent communication, listening, and interpersonal skills.
- Ability to work collaboratively with others and take initiative to improve patient care.
Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift up to 50 lbs. from floor to waist and push/pull with 30 lbs. of force.
- May be exposed to chemical and hazardous waste, blood, and body fluids, and communicable disease.
