Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Home Care team in Ocean County. As a LPN, you will provide high-quality nursing care to patients in their homes, working under the direction of a registered nurse. This is a full-time opportunity with a competitive benefits package.

Responsibilities
  • Provide nursing care to patients in their homes through assessment, development, implementation, and evaluation of care plans.
  • Comply with physician orders and chart patient information accurately and timely.
  • Assess patient acuity and communicate with healthcare team members as needed.
  • Document patient care and progress in the electronic medical record.
  • Teach patients and families about health needs and provide education on disease management.
  • Report patient problems and concerns to nursing management and primary nurse.
Requirements
  • Graduate of an LPN program or equivalent.
  • Six months of experience in home care or a related field.
  • Licensed Practical Nurse (LPN) certification.
  • AHA Basic Health Care Life Support HCP Certification.
