Job Title: Outpatient Licensed Practical Nurse

We are seeking a skilled and compassionate Outpatient Licensed Practical Nurse to join our team at WellSpan Health. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.

Job Summary:

The Outpatient Licensed Practical Nurse will be responsible for providing nursing care to patients in an outpatient setting. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare providers to ensure seamless patient care.

Key Responsibilities:
  • Obtain patient history and gather information to develop a comprehensive understanding of patient needs.
  • Perform specialized procedures, examinations, and tests as ordered and aligned with the Ambulatory Clinical Scope Document.
  • Ensure all patient care areas are adequately stocked with unexpired supplies and medications.
  • Relay information and facilitate communication between the provider and the patient concerning patient care.
  • Evaluate patient outcomes and meet with the manager and the hyperbaric physician to discuss patient treatment and adjust treatment as necessary.
  • Provide related education, counseling, ongoing review, and monitoring regarding diagnostic tests under the collaboration of the Physician/APP.
  • Assist in the management of patient care between visits and maintain patient records to ensure accurate documentation of the patient's medical treatment.
Requirements:
  • Diploma Program Certificate of completion (degree or diploma) from an accredited LPN program.
  • 1 year Clinical nursing experience.
  • Licensed Practical Nurse Upon Hire Required.
