Key Responsibilities:

Collaborate with the RN Case Manager and Utilization Management team to ensure seamless patient care and progression.

Resolve psycho-social barriers and support discharge needs for patients and their families.

Develop and implement effective care plans to meet the unique needs of each patient.

Requirements:

Master's degree in Social Work (MSW) or Master of Social Services degree (Client) from an accredited graduate school.

Licensed CSW or MSW as required by the state of Delaware.

Acute Care Social Work experience preferred.
