Role Overview:
The Senior Care Medical Practitioner (PCP) serves as a pivotal leader within our integrated care environment. We are committed to delivering high-quality care to our patients, ensuring that their health needs are met with compassion and expertise.
Key Responsibilities:
1. Assess and manage the health of center patients in line with established care standards.
2. Monitor patient care quality through data analysis and chart evaluations.
3. Facilitate coordination of patient services, including specialty referrals and home health care.
4. Actively engage with the care team during daily meetings, providing medical insights and support.
5. Collaborate with leadership to foster a cooperative practice environment.
Documentation:
Ensure timely completion of medical records, working alongside quality coders to enhance coding accuracy.
Compliance:
Adhere to policies and protocols set forth by Clinical Leadership and maintain compliance with all relevant licensing and accreditation standards.
Patient Care Focus:
Dedicate your time to direct patient care, encompassing both patient interactions and necessary administrative tasks.
Qualifications:
- Unrestricted medical license or willingness to obtain one in the relevant state.
- Graduate of an accredited MD or DO program.
- Strong communication skills and interpersonal abilities.
- Commitment to integrated team-based care.
- Basic computer proficiency, including familiarity with EMR systems.
Preferred Qualifications:
- Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Active DEA license and Medicare Provider Number.
- Experience in managing Medicare Advantage patients and understanding of coordinated care practices.
