Job Summary

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Care Manager. As a key member of our healthcare team, you will be responsible for providing high-quality care to our residents, while also supervising and guiding our nursing staff.

Responsibilities
  • Supervise and coordinate the care of residents, ensuring that their needs are met and their goals are achieved.
  • Lead and direct the nursing team, providing guidance and support as needed.
  • Monitor and document resident vital signs, treatments, and responses to care.
  • Administer medications and treatments as needed, while ensuring accurate and timely documentation.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
  • Identify and respond to patient needs, providing timely and appropriate interventions.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Perform RN assessments to identify and address resident needs, while maintaining accurate and up-to-date records.
  • Lead emergency situations, such as fire alarms or elopements, while ensuring resident safety and well-being.
Requirements
  • Current and active RN license in the State of New York.
  • Previous experience in geriatric or short-term rehabilitation settings, with a focus on Alzheimer's care.
  • Progressive nursing experience in a geriatric setting, with supervisory experience preferred.
  • Dedication to providing exceptional quality care to each resident, with a genuine interest in helping those in need.
  • Excellent communication, interpersonal, and customer service skills, with the ability to work well under pressure.
  • Able to work harmoniously with all staff members, while maintaining a patient and willing attitude.
  • Covid-19 vaccinations are encouraged but not mandated, with ongoing vaccination opportunities available.
