We are currently looking for several per diem and full-time LPNs for various continuous care roles available.
Position Overview:
In this role, your primary responsibilities will revolve around supporting the nursing care requirements of our patients as directed by the RN and outlined in the care plan.

Key Responsibilities:

  • Evaluates patient and family responses to care and monitors for changes in condition. Reports any changes appropriately to the RN case manager and secures orders for modifications in the medical treatment plan as needed.
  • Implements suitable preventative, therapeutic, and rehabilitative nursing techniques while maintaining communication regarding patient care with the physician, supervisor, and family as necessary.
  • Assists the RN and/or physician in executing specialized tasks related to end-of-life care.
  • Prepares clinical and progress notes promptly and accurately. Documents nursing observations, interventions, evaluations, and outcomes related to patient care management.

Qualifications:

  • Graduate from an accredited practical nursing program.
  • Current LPN licensure by the state board of FL.
  • At least 6 months of general nursing experience is preferred, including current experience in one of the following areas: hospice, home care, med/surgery, or critical care.
  • Strong nursing judgment and documentation capabilities.
  • Excellent verbal and written communication and interpersonal skills.
  • Current CPR certification is preferred.
  • Demonstrates sensitivity to the needs of patients and their caregivers (families, friends, and significant others).

Ability to work effectively as a member of an integrated healthcare team.
