Responsibilities:

  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with company policies and procedures.
  • Participate in ongoing education and training to stay up-to-date on best practices in physical therapy and home health care.
Requirements:
  • Current licensure as a Physical Therapy Assistant in the state of practice.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift 25 pounds and stand for long periods.
