MDS Coordinator Job Summary

We are seeking a skilled MDS Coordinator to lead our care management efforts at The Villas Senior Care Community. As a key member of our team, you will be responsible for ensuring the highest quality of care for our residents.

Key Responsibilities:
  • Lead the care management process by partnering with residents, families, physicians, and healthcare providers to provide comprehensive care.
  • Audit resident charts and care plans to ensure accurate documentation and compliance with Medicare and Medicaid guidelines.
  • Complete and transmit MDS assessments and reports in accordance with regulatory requirements.

Requirements:
