About the Role

We are looking for an experienced Healthcare Specialist to join our team at NYC Health + Hospitals. In this role, you will be responsible for providing high-quality patient care and working closely with other healthcare professionals to achieve the best possible outcomes.

Key Responsibilities
  • Patient Education: Develop and implement individualized education plans to promote healthy behaviors and disease prevention.
  • Care Planning: Collaborate with patients, families, and healthcare teams to create comprehensive care plans that address patients' unique needs.
  • Documentation: Maintain accurate and timely documentation of patient interactions, including progress notes and treatment plans.
Requirements

To be considered for this role, applicants must have:

  • A Bachelor's of Science degree in Nursing or higher from an accredited college or university, registered with the NYSED.
  • Demonstrated clinical competence and leadership traits, with a minimum of one year of clinical nursing practice as a medical/surgical, behavioral health, or correctional health nursing experience.
  • Basic Life Support Certification through the American Heart Association (AHA).
Estimated Salary

$93,011 per year, based on experience.
