Job Summary

We are seeking a skilled and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in developing and implementing personalized care plans for our patients. Your clinical expertise and excellent communication skills will enable you to effectively advocate for our patients and their families.

This is a full-time, Monday-to-Friday position with no weekends or nights. After a successful orientation period, you will have the option to work from home two days a week and in the office three days a week.

Key Responsibilities
  • Develop and implement personalized care plans for patients
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Communicate effectively with patients and their families to understand their needs and preferences
  • Monitor patient progress and adjust care plans as needed
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements

To be successful in this role, you will need:

  • A valid RN license and unrestricted certification in the state or a state with licensure reciprocity
  • Successful completion of our case management orientation program
  • A minimum of 2 years of experience in an acute inpatient setting or specialty area (e.g., ICU, ER, Oncology, NICU, Acute Rehab)
  • Excellent communication and interpersonal skills
  • Basic typing and computer knowledge with a minimum keyboarding speed of 35WPM
