Job Title: Behavioral Health Clinical Therapist

About the Role:

We are seeking a highly skilled and compassionate Behavioral Health Clinical Therapist to provide top-notch care to our patients. As a member of our multidisciplinary team, you will work collaboratively to achieve exceptional outcomes and model proficiency through precepting new team members.

Key Responsibilities:
  • Implement counseling/therapy for assigned patients in individual and group settings.
  • Interview and assess patients, and/or their family, caregivers, and/or legal representatives.
  • Determine, provide, and/or arrange for needed internal and external services/interventions.
  • Participate in case reviews/care conferences to evaluate progress.
  • Assist with discharge planning and processes, including identification of community-based resources, and provide referrals as part of the discharge plan.
  • Assess, collaborate, and coordinate patient's discharge planning needs and readiness for discharge with other members of the care team.
  • Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met.
