**Job Summary:**

We are seeking a skilled Physical Therapist to join our team in providing high-quality home health care services. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing interventions to improve patient well-being.

**Responsibilities:**

* Evaluate patient functional status and physical therapy needs
* Assist the physician in developing the therapy plan of care
* Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
* Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals

**Requirements:**

* Current Physical Therapy licensure in the state of practice
* Current CPR certification
* Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
