Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will have the opportunity to work in post-acute facilities in the area, providing high-quality patient care and making a meaningful difference in the lives of our residents.

Job Responsibilities:
  • Provide direct patient care, including administering medications, monitoring vital signs, and conducting intakes and assessments.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies.
Qualifications and Skills:
  • Current Connecticut state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of an approved LPN educational program.
  • High school graduation, GED, or equivalent.
  • At least 6 months of experience as an LPN or in a similar role.
  • Ability to lift a minimum of 50 lbs.
