Key Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Conduct thorough assessments of patients' physical and emotional needs, and make recommendations for care.
- Document patient progress and communicate effectively with healthcare teams.
- Current RN or LPN licensure in the state of MA.
- Minimum 1 year of experience in private duty nursing or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
