Hospice RN Case Manager

Reliant at Home is seeking a skilled Hospice RN Case Manager to join our team in San Antonio and surrounding areas. As a Hospice RN Case Manager, you will play a vital role in providing high-quality care to patients and their families.
Responsibilities:- Conduct comprehensive assessments of patients' physical, functional, psychological, and environmental needs.
- Apply specific criteria for admissions and re-certifications to hospice care to establish appropriate levels of care and patient eligibility.
- Develop and implement individualized plans of care to ensure quality and continuity of care.
- Consult with patients and families regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Provide training to other staff members as needed.
- Initiate preventative and rehabilitative nursing procedures as necessary.
- Apply concepts of infection control and standard precautions in coordinating and performing patient care activities.
- Prepare clinical and progress notes that demonstrate progress towards established goals.
- Ensure continuity of quality patient care delivered with appropriate documentation.
