About the Role

We are seeking a skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will provide comprehensive patient assessments and visits, as well as short-term crisis intervention and individual or family counseling.

Responsibilities
  • Conduct thorough patient assessments to understand their physical, emotional, and mental needs.
  • Provide scheduled patient visits to ensure continuity of care.
  • Offer short-term crisis intervention and individual or family counseling to support patients and their loved ones.
  • Utilize electronic medical records to document patient information and progress.
  • Serve as an advocate for patients and their families, ensuring their needs are met.
Requirements
  • Masters degree in Social Work is preferred, but a Bachelors in Social Work is also accepted.
  • Experience in Medical Social Work is desirable.
  • A background in the Hospice field is preferred.
  • Valid driver's license, car insurance, and reliable transportation are required.
  • You must be willing to travel throughout assigned service areas.
