Job Description

CivicMinds, Inc is seeking a dedicated and compassionate Registered Nurse to join our team as a Home Care Specialist.

About the Role
  • This is a full-time position based in Brooklyn, NY, requiring collaboration with Agency personnel to meet patient needs.

As a Registered Nurse, you will be responsible for evaluating patients referred for home care and determining their appropriateness for admission. You will establish a comprehensive home care plan, assess patient needs, and collaborate with Agency staff to ensure seamless service delivery.

Key Responsibilities:
  • Evaluate patients referred for home care to determine suitability for admission.
  • Develop personalized home care plans based on patient assessments.
  • Collaborate with Agency personnel to ensure timely and effective service delivery.
  • Communicate Agency policies and services to patients and family members during initial assessments.
  • Maintain accurate documentation and submit it promptly to the agency office.
Requirements:
  • RN with BSN required.
  • Current valid RN nursing license from NY.
  • A minimum of 1 year of nursing experience.
