About the Role

A skilled Physical Therapist is required to assess patients' strengths and weaknesses, devising treatment plans to aid patients in healing, recovering, and improving functional independence.

Key Responsibilities
  • Examine patients, implement treatment plans, and execute plans of care on a daily basis.
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
  • Establish treatment plans, collaborate with interdisciplinary teams, and assess and re-assess the need for modifications to plans of care and goals.
Requirements
  • Bachelor's or Master's Degree in Physical Therapy required, Doctorate degree preferred.
  • Licensed Physical Therapist in the state practicing in.
  • Completed state-regulated CEU requirements in the appropriate state.
  • Valid CPR license and experience/knowledge with EMR software preferred.
