Job Title: LPN Home Care Specialist

Job Summary:
Helping Hand Nurse, LLC is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality home health care services to our patients. As a LPN Home Care Specialist, you will be responsible for delivering one-to-one nursing care in the home, following a plan of care, and communicating with physicians about patient changes.

Responsibilities:
  • Provide personalized nursing care to patients in their homes
  • Follow established plans of care and protocols
  • Communicate effectively with patients, families, and healthcare teams
  • Participate in ongoing education and training to stay current with best practices

Requirements:
  • Current LPN license
  • Experience with pediatric instruments
  • Clean background and criminal record
  • CPR certification
  • Knowledge of computerized documentation and office software

Work Environment:
