We are seeking an exceptional Physician Team Leader (POD Leader) to join our clinical team. As a key resource and coach, you will empower your team to excel in clinical performance. Collaborate with advanced practice providers, build a cohesive team, and drive outstanding results.

Responsibilities
- Resource and coach for the clinical team
- Collaboration with advance practice providers
- Building a collaborative team

Requirements
- Experience in Geriatrics or complex patients preferred
- Active/unrestricted medical license and DEA
- EMR experience
- Active CPR Certification
- Completed an accredited residency program
- ABMS or AOA board certification

Preferred Qualifications
- Experience caring for patients in their home setting
