About Our Opportunity

We are seeking a highly skilled and compassionate Senior Clinical Care Coordinator to join our team at CenterWell Home Health. In this role, you will be responsible for developing and overseeing treatment plans in collaboration with physicians, evaluating patient needs, and coordinating care among team members to ensure optimal health outcomes.

Key Responsibilities:
  • Develop and implement comprehensive treatment plans that address the unique needs of each patient
  • Collaborate with healthcare professionals to ensure seamless coordination of care and timely completion of assessments
  • Evaluate patient progress and make adjustments to care plans as needed
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and high-quality care
Requirements:
  • Bachelor's degree in Nursing (BSN) or related field required; Master's degree preferred
  • At least 2 years of experience in clinical home care, with a minimum of 1 year in a leadership or management role
  • Current CPR certification and ability to obtain state-specific certifications as required
  • Excellent communication, interpersonal, and leadership skills
  • Knowledge of state and federal regulations governing home health services and ability to ensure compliance
  • A valid driver's license and reliable transportation for travel to patient homes and other locations as needed
Compensation and Benefits:

We offer a competitive salary range of $75,000 - $105,000 per year, based on experience and qualifications. Our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) retirement savings plan, paid time off, and opportunities for professional growth and development.
