About the Role

Key Responsibilities
  • Provide Direct Patient Care: Deliver evidence-based nursing care and patient education to ensure optimal health outcomes.
  • Care Management: Connect patients with specialists and follow up to ensure they receive the necessary care.
  • Population Management: Utilize population management tools to ensure patients receive optimal care based on industry-wide recommendations.
  • Phlebotomy and Laboratory Services: Perform phlebotomy services, in-house laboratory tests, and work with providers to determine when to send out specialty labs.
  • Vaccine Administration: Screen for vaccine-preventable diseases and administer primary care and travel-related immunizations.
  • Scheduling and Triage: Manage RN schedules and appropriateness of appointments, and provide general triage services to walk-ins and over the phone.
  • Quality Improvement: Participate in quality improvement programs to ensure the practice is performing efficiently and in accordance with Crossover's policies.
  • Health Technology and Analysis: Demonstrate proficiency in the use of healthcare analytic tools, systems, and analysis.
  • Population Health Programs: Manage population health programs and clinic outreach initiatives.
  • Nurse Resource and Training: Serve as a resource to other nurses and the care team, and participate in the training and onboarding of new nurses.
Requirements
  • Education: Graduate from an accredited school of nursing.
  • Experience: Minimum 3 years of clinical experience.
  • Licensure: Current Registered Nurse license in the state you are practicing.
  • Certification: Current BLS (Basic Life Support) certification.
Preferred Qualifications
  • Education: Bachelor of Science Degree in Nursing (BSN).
  • Experience: Outpatient care and urgent care experience.
  • Technology: Knowledge of Apple and Google-based technology or client technologies such as Microsoft or HP.
  • Skills: Passion for delivering a best-in-class patient care experience in an integrated patient-centered primary care model.
