Nurse Practitioner

PATH Integrated Health is seeking a highly skilled Nurse Practitioner to join our dynamic team of healthcare professionals. As a key member of our behavioral health team, you will play a vital role in providing high-quality patient care and contributing to our mission of delivering compassionate and comprehensive mental health services.
Key Responsibilities:- Assess and diagnose individuals and families, providing evidence-based treatment plans and interventions.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and patient outcomes.
- Develop and implement educational programs to promote patient empowerment and self-management.
- Prescribe medications and order laboratory screenings as needed, interpreting results to inform treatment decisions.
- Conduct thorough assessments of prior care and consult with previous providers to ensure continuity of care.
- Maintain accurate and detailed documentation of patient interactions and treatment plans.
- Participate in quality improvement initiatives and contribute to the development of best practices in behavioral health care.
We are seeking a Nurse Practitioner with a Master of Science in Nursing (MSN) degree from an accredited university and current licensure as a Psychiatric Mental Health Nurse Practitioner. A minimum of 3 years of nursing experience, including 1 year in a behavioral health environment, is required. Strong communication and interpersonal skills, as well as proficiency in electronic health records and telehealth platforms, are essential for success in this role.
