**Job Summary:**
As a Registered Nurse with Atrium Health, you will provide direct nursing care to hospice patients, including needs assessment and developing, implementing, and evaluating a plan of nursing care. You will provide direct patient care on an ongoing basis in accordance with the Nurse Practice Act and Hospice standards of care.
**Key Responsibilities:**
- Assess patient needs and develop individualized plans of care.
- Provide direct patient care, including administering medications, wound care, and other treatments as needed.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Document patient progress and communicate with families and caregivers.
**Requirements:**
- Associate's degree in nursing.
- Minimum two years of nursing experience. Hospice or home health experience and CHPN certification preferred.
- Licensed as a Registered Nurse in the state of Georgia.
