Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Yale New Haven Health. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our organization.

• Provide medication counseling to patients and families, ensuring they understand their treatment plans and medications.
• Conduct timely and accurate medication order reviews, considering patient characteristics, laboratory values, and vital signs.
• Develop and maintain effective interdisciplinary relationships with the healthcare team to ensure seamless patient care.
• Participate in patient-centered initiatives and ensure clinical continuity where applicable.
• Oversee medication distribution and dispensing processes, ensuring safe and effective medication delivery.
• Adhere to departmental policies, procedures, and guidelines, as well as state and federal regulations.
• Report medication events and near misses, and participate in quality assurance activities.

• Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.
• Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable.
• Active and unrestricted CT Pharmacist Licensure.
• Basic Life Support (BLS) certification or obtained within 6 months of hire.
• Immunization certification or obtained within 6 months of hire.

• Basic knowledge of pharmacology and therapeutics.
• Understanding of pharmacy systems of drug distribution and formulary management.
• Excellent verbal and written communication skills.
• Ability to work with others and provide education and training to pharmacy staff, students, and residents.
