Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Minotola, New Jersey.

About the Role:

We are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our clients in their homes. As a member of our multidisciplinary team, you will work closely with nurses, therapists, and other healthcare professionals to develop and implement individualized care plans that promote independence and improve the quality of life for our clients.

Responsibilities:
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help clients achieve their goals.
  • Collaborate with the healthcare team to revise and update care plans as needed.
  • Educate clients and their families on rehabilitative care and activities to promote healthy living.
  • Accurately document client care management and services provided.
Requirements:
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
